Discount Contacts is an out-of-network provider for Davis Vision, EyeMed, Spectra/United Healthcare, VSP, and most other insurance plans.

The general procedure for reimbursement for these insurance providers is as follows:

Fill out an out-of-network reimbursement form.
Include your itemized receipt with the form (Discount Contacts provides these with every purchase).
Mail, fax, or email the completed form to the corresponding vision insurance provider.

Some providers have their own online process. Make sure to check with your provider first to verify.
